Do you know how insurance claims work? Melbourne Netball does not instigate any claim so please read all the material below to gain an understanding of the process.

How it works:

Do you have private health cover of your own?Call them and tell them you are injured and ask them what can you claim. Ask them what do medicare then claim that they can not cover.
This will effect the dollar amounts and types that you are permitted to claim.

Do you have a medicare card?
This will enable you to receive some types of financial assistance AFTER you may be required to pay for some costs out of your own pocket.

Are there outstanding costs that neither of these two groups do NOT cover?
Well it may be worth you applying for an insurance claim via Jardine Sports Insurance.

What you must do:

1. Read the materials below.
2. Follow the link to download claim form
3. Keep all your original receipts to present for claiming
4. Keep a record of who you speak to and the details via the insurance company
5. Obtain contact insurance details via the links below for Jardine Sports

When you require a claim form follow the details below.

We have a direct link with Netball Victoria's website. You DO NOT require a Netball Victoria Player Registration Number. You must write "Licenced Agreement Player" instead. If you find that your claim is rejected in relation to this information, please email the Company Director immediately.

We request that all players also have private health cover. The is no such opportunity to 100% cover in a claim.

All injuries must be reported on the night. Even if an incident is slight, report it. We are not legally permitted to back date any incident. An accident report form must be filled out by your evening Coordinator for a claim to be processed. This report form must contain a witnesses to the injury. If you wish to lodge a claim you must keep all your original receipts from any medical treatments.

You will then proceed to the Netball Australia Website. Click on the 'Whats covered' icon on the left hand side of the page. Then click on the 'Making a claim' icon also on the left hand side of the page. Scroll down to clause 2.

It's here that you can download the insurance claim form. Make sure you read all the information.

Fill in the details and mail this to the Company Director at P.O.Box 2300 Richmond South 3121 with a self addressed envelope. It is filled in by the Company Director then mailed back to you.

You can then lodge the claim. A claim needs to begin lodgement within an eight week period.We suggest you make a note of the person you speak to at the Insurance Company, the date of the phone call and the time you called for your own personal records.
