Insurance Claim Procedure

Do you know how insurance claims work? Melbourne Netball does not instigate any claim so please read all the material below to gain an understanding of the process.

How it works: All insurance paperwork must readh the Melbourne Netball offie within 14 days. All claims must be lodged within 28 days of the date of injury. If you fail to do this your claim WILL be dismissed by the insurance broker. This is a requirement for their system.

1. Download Claim Form
2. Fill in your own personal details where asked
3. Get your acting Medical Person to fill in all the injury details
4. Once these are filled out mail ENTIRE claim form to:

1. We fill out our section and mail the entire form directly to the Insurance company
2. Insurance company calls you direct to open up an account with you. KEEP YOUR OWN RECORDS.
3. Do NOT send us your receipts. We are not permitted to handle them. Keep an extra copy for yourself
